HCA Healthcare is seeking a compassionate and skilled Home Health Physical Therapist to join our dedicated team. In this role, you will provide one-on-one therapeutic care directly in patients' homes, making a significant impact on their recovery and quality of life. This position requires travel to patient homes and flexibility to adapt to different environments. You'll work as part of a collaborative healthcare team to deliver personalized, effective therapy plans that respect the individual needs and environments of our patients. Assess patient condition by examining muscle strength, mobility, and functionality. Develop and implement tailored physical therapy treatment plans in accordance with physician recommendations. Educate patients and families on treatments, prevention strategies, and post-therapy care. Maintain accurate patient records and document patient progress. Collaborate with healthcare professionals to optimize patient care and recovery. Adjust treatment plans as necessary to meet patient needs and responses. Excellent knowledge of physical therapy principles and methods for rehabilitation Superb communication and interpersonal skills for effective patient and team interactions Ability to handle physical demands of the job, including transporting and assisting patients Critical thinking skills to adjust therapy plans as required Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Physical Therapy license CPR certified Valid driver’s license and reliable transportation as home visits are required
