Job Description /n NeuroRestorative, a part of the Sevita
family, provides rehabilitation services for people of all ages
with brain, spinal cord and medically complex injuries, illnesses
and other challenges. In a variety of locations and community-based
settings, we offer a range of programs, including vocational and
therapy programs, day treatments, and specialized services for
infants, children, adolescents, Military Service Members and
Veterans. /n LPN/LVN/n $34-39/hr Shift Differential! /n Full time
night shift /nSUMMARY /n The Licensed Practical Nurse is
responsible for providing prescribed medical treatment and personal
care services under the direction of a registered nurse (“RN”) or a
physician to address the medical or psychiatric needs of
individuals with developmental disabilities, acquired brain
injuries, substance abuse issues and/or emotional or behavioral
challenges in more than one site. Travels between multiple
locations to perform the duties and responsibilities of the role
and is accessible to staff at all locations during working hours.
Depending on the program, sometimes required to be accessible after
hours for consolation. /n ESSENTIAL JOB FUNCTIONS /n To perform
this job successfully, an individual must be able to satisfactorily
perform each essential function listed below: /n /n - Provides
basic care and medical treatment to individuals receiving services.
/n /n /n - Completes health assessments and provides input of
health diagnosis to RN or physician for the development of nursing
care plan. /n /n - Follows-up on all physician orders /n - Monitors
and ensures follow through on individual’s medical/dental
appointments. /n /n /n /n /n - Monitors and records medications in
a timely manner; /n /n - Acts as a liaison to pharmacy /n - Reviews
medication error forms, analyzes errors and makes recommendations
for procedural changes /n - Administers medication as required /n
/n /n /n - Oversees and monitors staff member who administer
medication. /n /n - Conducts quality assurance checks on health
care records. /n /n - Provides emergency coverage and/or care for
individuals receiving services. /n - Maintains confidentiality,
respects human rights and practices universal precautions in
accordance with Company policy. /n /n - Upholds policy and
procedure development related to health care. /n /n - Partners with
external agencies to coordinate services and ensure consistent
treatment. /n - Administers, tracks, and maintains log for staff
members’ and individuals’ testing. /n /n /n - Provides training to
staff members including but not limited to medication
administration, universal precautions and infection control. /n /n
/n - Provides training for individuals receiving services in topics
including, but not limited to self-medication, prescribed
treatments, sex education/STD prevention and smoking cessation. /n
- May be required to coordinate, organize and/or assist with
household activities such as light housekeeping and meal
preparation; transport individuals receiving services to planned
and/or necessary activities, appointments. /n - Travels between
multiple locations during work hours /n /n - Provides support and
is accessible to staff at all locations during working hours /n -
May be required to be accessible after hours for consolation /n /n
- Participates in on-call system as needed. /n /n /n - Performs
other related duties and activities as required. /n /n SUPERVISORY
RESPONSIBILITIES /n /n - None /n /n Minimum Knowledge and Skills
required by the Job /n The requirements listed below are
representative of the knowledge, skill, and/or abilities required
to perform the job: /n /n Education and Experience: /n /n -
Associate’s Degree in Nursing or related field, or a graduate of an
accredited LPN program /n - One year experience working with
population served in a nursing capacity /n /n Certificates,
Licenses, and Registrations: /n /n - Current state LPN License /n -
Current driver's license, car registration and auto insurance if
driving on the behalf of the Company or providing transportation to
individuals receiving services. /n - Other certifications as
required by state licensing regulations such as CPR/FA and HIV/AIDS
certification, BBP, and ACT training. /n /n Blood Borne Pathogen
Category: /n /n - Job requires performance of duties that involve
potential for exposure to blood, body fluids, or tissues. Tasks
that do involve exposure are an expectation of employment. /n /n /n
Physical Requirements: /n /n - Heavy work. Exerting up to 100
pounds of force occasionally, and/or up to 50 pounds of force
frequently, and/or up to 20 pounds of force constantly to move
