Home Health Care is a provider of pediatric/adult, occupational,
physical, speech and feeding therapy services. Our Therapist
provide the highest level of pediatric and adult care. We offer
care services ranging from specialty therapies, applied behavioral
analysis, skilled nursing, to basic personal care assistance. We
are seeking a passionate Physical Therapy Technician who loves
working with the pediatric or adult population to provide
home-based therapy services. We are looking for clinicians who live
in or nearby the following areas: Pueblo, CO Duties: Assists
Physical Therapist (PT) with providing consumer services by using
observational skills; assists with documentation of the care
provided. Assists with documentation of evaluations and development
of a rehabilitative plan of care as required. Assists in the
preparation of clinical and daily progress notes; all notes are
reviewed by the PT and submitted per agency policy. As directed,
physically assists the Physical Therapist with performing routine
strengthening and therapeutic exercises and assists the consumer
with the use of self-help devices. Observes and records activities
and findings in the clinical record as instructed by the Physical
therapist. Completes documentation on the same day as the visit.
Attends rehabilitation department staff meetings and case
conferences as required for the Physical Therapist. Organizes the
daily schedule for the Physical Therapist and confirms on a weekly
basis the scheduling of supervisory visits for the Assistants
supervised. Drives the Physical Therapist to all scheduled visits
and meetings using the company car. Help schedule maintenance
visits for company car accordingly. Assumes other duties deemed
appropriate and necessary by the PT and as designated by Agency
Supervisors/Management. The ability to drive and the ability to
access consumers’ homes which may not be routinely wheelchair
accessible are required. Hearing, eyesight and physical dexterity
must be sufficient to assist the Physical Therapist accordingly. On
occasion, may be required to bend, stoop, reach and move consumer
weight up to 250 pounds; lift and/or carry up to 30 pounds. The
ability to communicate well, both verbally and in writing in
English is required Requirements: 1. Rock-solid reliability &
safety This is non-negotiable. Excellent driving record and calm,
defensive driving habits Punctual and consistent —no last-minute
chaos Comfortable navigating unfamiliar neighborhoods and tight
schedules Good situational awareness (traffic, weather, parking,
accessibility) 2. Strong verbal communication skills Because vision
isn’t the primary channel here. Can describe environments clearly
and efficiently (room layout, obstacles, patient positioning) Gives
concise, accurate feedback without over-interpreting Listens well
and checks for understanding Comfortable reading forms, addresses,
signs, and instructions out loud when needed 3. Respectful
understanding of blindness (not savior energy) This is huge. Treats
the PT as the lead clinician , not someone to “help” or manage
Knows when to offer information vs. when to step back Comfortable
taking direction from a blind professional No awkwardness, pity, or
over-helping 4. Clinical support competence They don’t need to be a
PT, but they do need to be solid. Willing and able to learn PT
terminology and routines Can assist with: Equipment setup and
breakdown Patient transfers (within training/scope) Counting reps,
timing exercises, documenting basic data Follows instructions
precisely and respects scope of practice 5. Professional boundaries
& patient rapport They’re in people’s homes—this matters. Polite,
calm, and emotionally steady Good bedside manner without becoming
the “main character” Maintains confidentiality and HIPAA awareness
Reads the room well with patients and caregivers 6. Adaptability &
problem-solving Home health is never predictable. Can think on
their feet when: A house layout is unsafe A patient is late or not
home Equipment is missing or broken Stays calm and
solution-oriented instead of flustered 7. Physical capability The
job is real-world physical. Able to lift, carry, and move equipment
Comfortable standing, walking, and assisting throughout the day
Good body mechanics and injury awareness 8. Personal traits that
make the partnership work These often matter more than the resume.
Trustworthy and discreet Patient, steady, and emotionally mature
Takes feedback well and learns quickly Genuinely interested in
healthcare and helping people function better Nice-to-have bonuses
Not required, but gold if you find them: Background as a PT aide,
EMT, CNA, or rehab tech Experience working with people with
disabilities Familiarity with accessibility tools or orientation &
mobility concepts Basic medical documentation or EMR exposure High
school diploma or equivalent Valid Driver's License CPR
Certification Home Care experience preferred
