Psychiatric Nurse Practitioner works closely with Primary Care
Medical Providers, Behavioral Health Providers and Case managers to
support patients with severe and ongoing mental health needs. This
role provides psychiatric evaluations, medication management, and
follow-up care, primarily at the in our Housing Supportive Services
Program and follows a community/home outreach model. The position
focuses on delivering integrated, trauma-informed care to
individuals experiencing homelessness or housing instability and
may also provide services at the Stout Street Clinic. NOTE: In
order to ensure that our consumers receive the best possible care
the candidate chosen to fill this position will be required to
complete our internal credentialing and privileging process prior
to a start date. The credentialing and privileging timeline is
dependent on the selected candidate's submission of documents
necessary for clearance. Please note that a start date will be
scheduled once credentialing and privileging is complete.
Additional Requirement Valid driver’s license required. This
requirement may be waived, if necessary, based on overall candidate

hire. Essential Job Functions Works closely with Housing Support
Services teams to bring services to the client, often visiting them
at their housing site to provide psychiatric care, crisis
intervention, and medication management. Provides direct
psychiatric care, including new patient evaluations, follow-up
care, and care coordination on both a scheduled and walk-in basis.
Collaborates closely with Behavioral Health Providers (BHPs) to
provide psychiatric consultation to Primary Care Medical Providers
(PCMPs). Provides urgent and crisis psychiatric care. Conducts
diagnostic assessments consistent with DSM-5 and/or ICD-10, taking
into account patients’ economic and housing circumstances, medical
conditions, and substance use disorders. Orders and interprets
diagnostic tests as clinically indicated. Develops treatment
recommendations addressing both mental health and substance use
disorders, considering patients’ trauma histories, economic
conditions, and housing status. Coordinates care with other
providers within CCH and external partner agencies. Participates in
assessments of patient risk and facilitates transfer to inpatient
care when clinically indicated; may initiate Mental Participates in
regular supervision with the Psychiatric Director or designee.
Performs other duties as assigned. Qualifications Summary
Preference for candidates whose past experience includes
significant psychiatric experience. Proven clinical competence in
the provision of psychiatric health care in a community setting.
Demonstrated experience, competence, and sensitivity in working
with homeless and at-risk individuals and families. Education A
Master of Science degree in Nursing from a program accredited by
the American Nurses Association or the American Association of
Nurse Practitioners. Current registration in the State of Colorado
as an Advanced Practice Nurse. Certification as a Psychiatric Nurse
Practitioner from the American Nurse’s Credentialing Center.
Prescriptive authority from the Colorado State Board of Nursing.
Consideration will be given to candidates who have completed all
the requirements for this authority except for supervisory hours.
Certificate of Added Qualification in Psychiatry required. WHERE A
CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL
RELEVANT YEARS OF EXPERIENCE Qualified candidates are eligible for
the NURSE Corps Loan Repayment Program which offers payment of 60%
of unpaid nursing education debt in exchange for a two-year
commitment at a NURSE Corps approved site with an option to extend
to a third year for an additional 25% of the original balance.
